HL7 Version 2 Implementation Guide: Laboratory Value Set Companion Guide, Release 2 - US Realm
DESCRIPTION
HL7 has developed multiple Implementation Guides (IGs) for the laboratory domain, supporting US Realm requirements. These IGs cover a broad spectrum of interoperability; in order to harmonize vocabulary requirements across the IGs, HL7 Orders & Observations Work Group created a new approach, publishing a separate Companion Guide in order to document how vocabularies are used across the US Realm laboratory IGs.
This Value Set Companion Guide provides detailed value set definitions for each component and field where a coded value is required to be communicated. The separation of vocabulary from the IG enables the declaration of a minimum release version to be associated with the release of a Laboratory US Realm Implementation Guide such that the value sets can be versioned over time without requiring a revision of the referring Implementation Guide. Thus the value set version stated at the time of Implementation Guide publication OR NEWER can be used to satisfy the requirements of this IG at the time of implementation.
The Value Set Companion Guide includes introductory material and a master index that links to a spreadsheet for each value set. Each spreadsheet describes the detailed usage requirements for implementations intending to be conformant to this guide, i.e., the required, permitted, and excluded values. These concepts are fully detailed in the Companion Guide.
In the case of a single fixed value that will always be constant, e.g., the value of MSH-12.1 (Version ID.Version ID), the value is constrained by a Conformance Statement in the IG itself; it is also listed in the master index but no detailed spreadsheet is provided. Other code systems such as LOINC, SNOMED CT, USPS, etc. are also listed with additional constraints noted.
Note: this guide does NOT address coordination of use of updates between trading partners. See the Value Set Companion Guide for full details on how values sets are created, managed, and the scope and expectations for use.

DEVELOPMENT BACKGROUND
The HL7 Companion Guide is the result of collaborative efforts between HL7 and the Office of the National Coordinator (ONC) Standards and Interoperability (S&I) Framework Laboratory Results Interface (LRI), Laboratory Orders (LOI) and the electronic Directory of Service (eDOS) Initiatives. By consensus this guide documents the definition, management and use of code systems and their contents (i.e., a value set) to meet the requirements of the US Realm Laboratory suite of Implementation Guides.
The Laboratory Results Interface Initiative identifies the requirements, defines specifications and standards to provide implementation guidance for electronic communication of a laboratory’s electronic Directory of Services to an EHR, the electronic ordering of a laboratory test, and the reporting of laboratory test results to ambulatory care providers in the US Realm. These guides have been designed and developed to offer an end-to-end process for “out of the box” interoperability with minimum pre-coordination between trading partners.
