HL7 FHIR® Implementation Guide: Multiple Chronic Conditions (MCC) eCare Plan, Edition 1 - US Realm

DESCRIPTION

The implementation guide represents clinical and social data elements within the FHIR Care Plan Resource. It defines the FHIR resource relationships necessary to support care planning and reuses or defines FHIR queries to achieve an aggregated view within the Care Plan structure. The intent is to facilitate data aggregation from all systems where patients receive care, incorporating patient provided data and support care coordination. It provides an EHR agnostic, holistic view of care and a solution to the interoperability concerns among EHRs to write back or pull data into their EHRs.

See the standard at http://hl7.org/fhir/us/mcc/STU1.

DEVELOPMENT BACKGROUND

Care plans began as a teaching tool for nursing students to document patient conditions and associated care, evolving into a requirement for most patients by various bodies, transitioning from a nursing-based plan to an interdisciplinary tool. While major vendors have electronic care plan modules, their implementation and use are unclear, and few meet exchange standards. The first HL7 standard for care plans was the C-CDA Care Plan, influencing the FHIR Care Plan Resource and Integrating the Healthcare Enterprise (IHE) Dynamic Care Planning (DCP) profile. The Office of the National Coordinator (ONC) has regulated that goals and health concerns be accessible for exchange; often, this is met by free text. The DCP profile, using FHIR Resources, structures care planning and sharing, but lacks specific condition representation and has no known real-world implementations. This Implementation Guide (IG) aims to be the first fully defined HL7 FHIR IG, using FHIR Care Plan resources to represent a dynamic care plan and aggregate patient care data and outcomes.
