HL7 FHIR® Implementation Guide: US Patient Care Summary, Edition 1 - US Realm
DESCRIPTION
The US Patient Care Summary (US PCS) Implementation Guide defines a standard way for US healthcare systems to create and share a concise digital summary of a patient’s most important health information, like medications, allergies, and problems. This better informs clinicians during care transitions. It is built using modern FHIR standards and reuses existing US Core profiles, packaging them into a single, consistent document that can be shared with caregivers or family. It includes both a text-based view and computer-processable coded information.
Its purpose is to enable safe, fast, and consistent information sharing across organizations and countries in situations like care transitions or unplanned encounters. The guide aligns with the HL7 universal realm International Patient Summary and adapts it to usage in the United States. The US-PCS serves as a bridge from older medical summary formats toward a more modern, streamlined approach.

BENEFITS
The benefits of the US-PCS include:
- Improve Care Transitions: Provides a concise, clinically relevant patient summary that helps clinicians quickly understand a patient's key health information during referrals, transitions of care, and unplanned encounters.
- Leverage Existing U.S. FHIR Standards: Reuses widely adopted US Core profiles and USCDI-aligned data elements, allowing organizations to build upon existing FHIR investments rather than creating new data models and workflows.
- Minimize Information Overload: Focuses on a core set of high-value clinical data rather than transmitting large, unfiltered clinical records, helping clinicians identify important information more efficiently.
- Conform to International Guidance: Aligns with and conforms to the HL7 International Patient Summary (IPS), supporting cross-border healthcare, patient mobility, travel, and care delivery across international jurisdictions.
- Guide Domestic Implementation: Provides guidance on the relationship between U.S. and international standards and terminologies, along with implementation artifacts that support both human-readable and machine-processable exchange.
- Accelerate FHIR-Based Document Exchange: Provides a standardized FHIR document format that can be exchanged today while healthcare organizations transition from legacy document-based exchange approaches, such as CDA, to modern FHIR-based interoperability.
IMPLEMENTATIONS/CASE STUDIES
Major electronic health records (Epic, MEDITECH) have launched the capability to produce IPS-conformant patient summaries using FHIR data aligned with US Core. Domestic health exchange (Washington Health Summary) and consumer apps (CommonHealth) have also launched patient summary sharing capabilities in the US market leveraging existing FHIR endpoints and profiles.
