Collaborative CDA Template Review Process Pilot

1437
Investigative Project (PSS-Lite)

White Paper

Details

This pilot project will explore using a collaborative review process, performed by resources from across a small set of domain workgroups, to facilitate collaboration CDA template assessment. The selected resources also will be engaged in working on development of the corresponding FHIR resources. The objective is to understand what is involved in working across workgroups to facilitate collaboration on CDA template assessment.

The project is investigative, in that it intends to investigate and document the identified changes needed to align and harmonize the templates, but it does not include doing the work to address the changes.

Once the agreed upon list of changes has been identified, a separate project or projects will be initiated to address making the needed changes. This will be handled as a two-step process and this project covers the first step only. In the future, this type of work could incorporate collaboration across other work groups such as Patient Administration, PHER, due to their expertise with templates such as record target and immunization activity.

This project will explore a cross-workgroup collaboration model that establishes a way to perform detailed review of CDA templates in order to identify issues and document change requirements that will improve and inform the creation of CDA templates and align them with ongoing FHIR resource development work.

The following CDA R2.0Templates and value sets are in scope for piloting this the collaborative review process:

1. For PC:

- C-CDA R2.1 Problem Concern Entry and all encompassed templates/value sets

- IPS Problem Concern Entry and all encompassed templates/value sets

- C-CDA R2.1 Allergy-Intolerance Concern Entry and all encompassed templates/value sets

- IPS Allergy and Intolerance Concern and all encompassed templates/value sets.

2. For FM:

- C-CDA R2.1 Coverage Activity Entry

- C-CDA R2.1 Planned Coverage

- C-CDA R2.1 Policy Activity Entry

3. For Pharmacy:

- C-CDA R2.1 Medication Activity Entry

- C-CDA R2.1 Planned Medication Activity

- IPS Medication Statement

- Pharmacy UV Medication Statement

- Pharmacy UV Medication Order

Many of the CDA templates have been developed within a single Work Group. The CDA template design process has not drawn as much active participation from other workgroups. As a result, the templates may not align optimally.

Recently the collaborative specification approach used by FHIR has increased the interest and profiling skills available in the domain expert workgroups.

The need to transform information between CDA and FHIR has also increased. This is increasing the need for CDA Templates and FHIR Resources/Profiles to be aligned. HL7 is also calling for greater alignment among its product families.

The CDA Management Group sees a need to actively encourage collaborative review of CDA templates across SDWG and the domain-expert Workgroups responsible for the corresponding FHIR Resources. This project will address the need to foster these new cross-workgroup collaboration patterns and will investigate what insights are gained from this type of multi-disciplinary review.

A confluence page has been created to track progress for this pilot project. PC has created a page for the Clinical Status value set review/request. That space is referenced by the confluence page for the overall pilot. A wiki page has also been created

The need to transform information between C-CDA and FHIR has also increased. This is increasing the need for C-CDA Templates and FHIR Resources/Profiles to be aligned. HL7 is also calling for greater alignment among its product families. ONC’s focus on USCDI data classes is also driving alignment of information representation across HL7 product lines for specific data elements

The focus will include both US Realm and international perspectives, but the focus will be on analyzing the C-CDA R2.1 templates in scope for the project. For example, analysis of the C-CDA R2.1 Medication Activity template may include assessment of its alignment with UV realm templates designed for the same purpose. Clarifications about the expected relationship between UV and realm-specific templates could be included in the paper.
