Implementation Guide

Details

Prescription Drug Monitoring Programs (PDMPs) are databases deployed in each US State to track controlled substance prescriptions in a state. PDMPs provide health authorities timely information about prescribing and patient behaviors.

This project will develop a US Realm FHIR Implementation Guide to support PDMP clients (prescribers, pharmacies, etc.) requesting a patient's dispensing records from PDMPs using FHIR.

This IG will be based on FHIR US Core 6.0.0 STU6 and FHIR 4.0.1 R4.

This IG will include data mapping to other standards used in the eRx/PDMP workflow - NCPDP SCRIPT, ASAP, PMIX/NIEM - as appropriate. This IG will not 'replace' any of these standards.

This project will address outstanding issues from 2020SEPT HL7 FHIR IG: Pharmacy; Prescription Drug Monitoring Program (PDMP), Release 1- US Realm.

This project builds upon, and replaces, previous PDMP work in Projects 1393 and 1291.

This FHIR implementation guide will use the US Core profiles. If this FHIR implementation guide is unable to use a US Core profile, we will follow the Cross Group Projects WG's variance request process and provide the US Realm Steering Committee an approved rationale for deviation in the implementation guide where applicable.

The PDMP cycle of information involves 3 players

1. Prescribers write the prescriptions, review related PDMP records, and send the script to the pharmacy

2. Pharmacies receive the prescription, process it, review related PDMP records, dispense the medication to the patient, and report the dispensation to the PDMP

3. PDMPs receive the dispensation information from the pharmacies, and respond to queries from prescribers and pharmacies for PDMP records.

Sending the script from the prescriber to the pharmacy is accomplished using NCPDP SCRIPT (SCRIPT 2017071)

Sending the dispensation from the pharmacy to the PDMP is accomplished using ASAP Standard for Prescription Drug Monitoring Programs (4.1, 4.2, 4.2a and 4.2b, soon adding 5.0)

The query from the prescriber or pharmacy to PDMP(s) is not currently standardized. Depending on the PDMP implementation each state, queries may be done via a web portal or a proprietary interface.

This project will created a FHIR Profile to standardize the PDMP records request/response.
