FHIR Implementation Guide

Implementation Guide

Details

This project supports the National Healthcare Safety Network (NHSN) in their efforts to modernize public health reporting through the use of FHIR quality reporting standards. This project will result in one or more FHIR implementation guides (IGs) defining standards for reporting to NHSN, building on existing work such as the Quality Measure Implementation Guide and Data Exchange For Quality Measures Implementation Guide efforts.

Note: Carol Maccumber is listed as the vocabulary facilitator, but she will be working closely with Shelia Abner who will support that role from the NHSN perspective.

NHSN reporting, while very successful over the years, needs to modernize. The long cycle of ballot, publish, then wait years for push implementations doesn't work in the realigned post-pandemic world. NHSN needs to get live data and be able to adjust criteria in near real time.

This project will move NHSN off of static one-off implementation guides for specific purposes, and onto a quality measure driven approach, where this project defines the overarching framework (based on the existing CMIG and DEQM implementation guides) and some baseline profiles, value sets, etc. for NHSN. Once established, NSHN can then create digital quality measures (dQMs ) with related value sets, CQL libraries, and other artifacts. that comply with this framework for specific use cases.

The dQMs are dynamic, executable artifacts that are not a balloted part of this specification. Thus, the criteria in compliant measures can be adjusted as needed without waiting for a year's long ballot/publish/implement cycle.

Also, this project focuses on leveraging data compliant with existing EHR FHIR APIs wherever possible, such as US Core. While we certainly hope that QI Core compatible content will become widely available from EHR FHIR APIs, this project does not require QI Core as a minimum baseline for data capture.

This project will require coordination between NHSN and other branches of CDC, along with CMS.
