Clinical Quality Language (Jira PSS-2409)

Ballot - STU to Normative

Create New Standard

Details

Clinical Quality Language has been a published mixed normative/trial use specification since April 2020. In that time the language has been used to represent clinical logic in multiple domains in the healthcare industry including quality measurement, decision support, public health reporting, chronic disease surveillance, and clinical guidelines. In addition, multiple vendors have implemented CQL engines and related tooling.

This project will incorporate feedback from across stakeholders and implementation experience to address issues that have been encountered, as well as add new capabilities that have been requested.

Specifically, the project will focus on:

1) Enhancing the test suite included in the specification to close gaps in testing coverage, address inconsistencies between the tests and the specification, and support demonstration of engine conformance

2) Adding capabilities to the specification based on implementer and stakeholder feedback, including:

support for default comparison precision

support for uncertainty as a type

additional terminology operators (~in and ~contains)

enable consistent error handling and reporting

formalizing JSON serialization of ELM

3) Consider what trial-use aspects of the specification can be considered for normative status

This project will address industry needs for standardizing CQL implementations for quality improvement including decision support and quality measurement, as well as other domains where CQL has been used such as burden reduction and public health reporting.

Feedback from stakeholders such as The Digital Quality Implementers' Community (DQIC), an initiative that has brought together several CQL engine developers to provide feedback on the specification and support the maturity of CQL. That initiative has identified through testing at connectathons, use cases where supported functions either cannot be tested, or the test results are divergent among different engine developers. These enhancements are crucial for ensuring accurate and effective clinical decision support and quality measurement.
