FHIR Implementation Guide

Create New Standard

Details

This project will develop and maintain a U.S. International Patient Summary (US IPS) Implementation Guide based on FHIR, aligned with US Core and USCDI. The US IPS will align with the international IPS standard while incorporating U.S.-specific clinical and regulatory requirements. The IPS, by design, is a minimal, non-exhaustive set of essential clinical data that is specialty-agnostic, condition-independent, and immediately usable by any clinician. Its core purpose is to provide a concise, trusted summary of key patient information, such as allergies, medications, and problems, to support unexpected or unscheduled care, including emergencies or accidents. Although optimized for urgent, unplanned encounters, the IPS can also support planned care, such as travel, citizen mobility, or cross-organizational care pathways. The U.S. version will preserve these foundational principles.

As part of this work, the project will also examine vocabulary and terminology challenges that affect cross-border interoperability to ensure semantic consistency and safe interpretation of patient summary data when exchanged internationally.

Additionally, this project will include guidance on the relationship to the existing CDA based CCD exchange.

The project will include testing and validation through HL7 Connectathons, pilot implementations, and the development of tooling to support conformance. Deliverables will include the US IPS Implementation Guide, validation and testing artifacts, and a defined maintenance process to support ongoing updates.

This FHIR implementation guide will use the US Core profiles. If this FHIR implementation guide is unable to use a US Core profile, we will follow the Cross Group Projects WG's variance request process, and provide the US Realm Steering Committee an approved rationale for deviation in the implementation guide where applicable.

The U.S. healthcare system requires a modern, streamlined patient summary that builds on the value delivered by the long-standing Continuity of Care Document (CCD) while preparing for a transition to FHIR-based document exchange. The CCD has served as a critical national asset for more than a decade, enabling broad adoption, supporting certification programs, and facilitating patient summary exchange across diverse settings. However, as global adoption of the International Patient Summary (IPS) accelerates, and as U.S. interoperability increasingly shifts toward FHIR APIs, a forward-looking approach is needed.

A US-specific, IPS-aligned FHIR Patient Summary will facilitate interoperability with international partners, reduce information overload and variability seen in CCD implementations, and leverage modern FHIR capabilities to support real-time exchange and usability. This work does not replace the full capabilities of the Consolidated Clinical Document Architecture (C-CDA) but establishes the foundation for a future transition of clinical documents — ensuring continuity for current implementations while charting a path toward a more modern patient summary aligned with global best practices.

The rapid global adoption of the International Patient Summary (IPS) is reshaping expectations for cross-border, interoperable patient information. Countries across Europe, Asia-Pacific, and the Americas are implementing IPS-aligned solutions to support emergency care, travel medicine, cross-jurisdictional health services, public health response, and modern digital health ecosystems. As reflected in the growing number of national implementations showcased worldwide, the IPS has become a foundational global asset for safe, timely, and semantically consistent patient information exchange. These international advancements create a strong external driver for the United States to establish a nationally tailored, IPS-aligned patient summary that maintains global interoperability while addressing U.S. clinical, regulatory, and technical realities. Aligning with this global momentum ensures that U.S. healthcare remains connected to an emerging worldwide framework, supports care for U.S. travelers abroad and internati
