FHIR Implementation Guide

Revise Current Standard

Details

Develop a FHIR implementation guide enabling consumers to use a FHIR-enabled app to learn the cost, insurance coverage and related information for medications they've been prescribed. Specifically…

  • how a medication will be covered by their insurance, including out of pocket costs and any coverage restrictions or requirements that might apply
  • cash and discount pricing available for the medication
  • available patient assistance programs and other information related to the medication

The sources of this information may include the patient's insurance company (and/or its pharmacy benefit manager), providers of medication pricing information, manufacturers or other facilitating entities.

This specification is, in part, an adaptation of the existing NCPDP Real-time Prescription Benefit (RTPB) standard which is designed for use between provider systems and payers. This consumer-facing IG intends to enable consumers to access information that is consistent with what is available to their providers through the NCPDP standard--while also providing cash/discount pricing and related information of interest to patients.

(Note: This PSS replaces the original 2019 PSS captured in Confluence)

The final Medicare Part D rule for 2020

(CMS 4180-F) requires all Medicare Part D plans to provide members with information on lower-cost therapeutic alternatives and to implement a real-time pharmacy benefit tool (RTPB) by 2021.

Subsequent state legislation has expanded these requirements:

  • California (SB 1419, 2024): Requires health plans to support standardized FHIR APIs for real-time pharmacy benefit transactions, aligning with CMS interoperability and Patient Access API frameworks.
  • Texas (HB 3359, 2024): Establishes a similar mandate for payers and PBMs to provide FHIR-based RTPB API endpoints, ensuring prescribers and consumers can access accurate, real-time cost and coverage information at the point of prescribing or dispensing.

These new laws make FHIR API implementation mandatory for certain payers and PBMs beginning in 2025, reinforcing the national transition away from proprietary RTPB formats toward FHIR R4-based interoperability that supports both prescriber-facing and consumer-facing workflows.

NCPDP members and leadership will be part of the group collaborating on this effort. However, we expect that the resulting IG will be balloted only through HL7.
