Details

This informative Implementation Guide (IG) will describe how to represent and share data related to Standardized Clinical Assessment and Management Plans (SCAMPs) using HL7 FHIR. SCAMPs are clinician-developed care pathways designed to reduce unwarranted practice variation and improve outcomes by combining standardization with a learning health system approach that actively encourages and evaluates clinically reasonable deviations.

The goal of this IG is to support interoperable, standards-based exchange of SCAMP data, including recommended care, actual care delivered, and documented reasons for deviation. It leverages the HL7 MedMorph Reference Architecture to automate reporting and facilitate population-level analysis. This IG specifically targets the use case of anticoagulation management in patients receiving extracorporeal membrane oxygenation (ECMO), with planned generalizability to other SCAMPs.

This project supports the transition from (or complementarity with) Value-based Purchasing to Pay for Transformation (P4T).  P4T is a set of principles and practices based on the belief that finance is a tool to enable and sustain transformation in care delivery.  P4T ensures that providers have the necessary evidence, infrastructure, workforce, and systems to be held accountable for care and resource utilization.  The SCAMPs infrastructure consists of a common community to pool resources, to define the transformation target, to define "sound practice" based on current experience, and then to iteratively and dynamically generate and evaluate evidence during the delivery of care.  "Positive Deviations" from sound practice are local innovations anticipated to improve outcomes that serve as hypotheses for analysis by the common community.  Substantiated hypotheses are then embedded in a refined version of "sound practice" and the cycle repeats.  The FHIR-enabled SCAMPs process is intended to supplement the existing evidence generation ecosystem, which tends not to work well many populations and clinical settings, including pediatric populations and resource-limited settings.
